Since health or medical insurance reimbursement is affected by many factors, Pelikan Technologies, Inc. makes no representation or guarantee that a patient will be successful in obtaining insurance reimbursement or any other payment.

Lancet Disk Reimbursement
For most insurance plans, you can claim coverage for your Pelikan Sun lancet disks. Take the following steps:

1. Download your insurance provider’s Claim Reimbursement form from the plan website. You may have to login as a plan member to find this link.
2. Complete the patient information details on the form. In the claim description/explanation section, simply write: A4259 Lancets – Box of 100.
3. Include a copy of your sales receipt from Pelikan Technologies
4. When requesting reimbursement for multiple boxes of lancets, be sure to include your physician’s prescription.

Pelikan Sun Lancing Device Reimbursement
